Patient Information – Requesting Medication issued by a Hospital
If you are contacting us regarding a medication which has been added or amended by the hospital, we would like to explain the process to you.
Any such changes to medication take two to four weeks to action by the Surgery.
First the consultant dictates a letter to his secretary. This is then typed and it goes back to the consultant for approval. The letter is posted via internal mail; it arrives at the surgery and is scanned in by our admin team. It is then forwarded to the practice pharmacist for medication changes. All this takes time, and making requests outside of this window puts extra pressure on our GPs and admin team.
A prescriber should only change medication if they have either assessed a patient themselves, or have clear written evidence that another clinician has assessed them.
If your consultant or their team requests you change your medication, they will need to issue you with a prescription on the day to bridge the gap until we can take over.
